To enter Origami By Children, the child must be 18 or younger as of January 31 of the participation year. Models must arrive no later than January 31 of the participation year. Limit one model per child.
Please print the PDF of this form received in your entry form confirmation email and have it signed by a Parent or Guardian. The signed form MUST be included with your model or it will be disqualified.

OrigamiUSA is not responsible for models damaged in transit. Carefully package your model(s) in a sturdy envelope or suitable box with plenty of protective filler. Do not use packing peanuts. Bubble wrap or soft crumpled paper is preferred.
All entries become the property of OrigamiUSA. All participants will be informed of their standings in the competition within two months of the judging. The decisions of the judges are final.
