Nomination due date: March 15, 2019
This form is the application for a young person to attend either the OrigamiUSA Annual Convention at St. Johns University in Queens, New York in June, 2019 or the Pacific Coast OrigamiUSA Convention (PCOC) in Portland, Oregon in September, 2019. Anyone may make nominations including young people for themselves. Any nominee must be 18 years or younger as of June 21, 2019 (for Annual Convention) or September 13, 2019 (for PCOC).
